How to Apply

Healthy NY Enrollment Restriction

For coverage beginning on or after January 1, 2012, applicants for Healthy NY coverage will only be able to enroll in the High Deductible Health Plan (HDHP). Current enrollees will not be impacted by this restriction. Please visit the links below for more detailed information on the change.

An important step in applying for Healthy NY is choosing the HMO which will provide your coverage. Healthy NY is offered by all HMOs in New York State, and the Healthy NY benefit packages are the same regardless of which HMO you choose.

You must apply for the Healthy NY coverage directly to the health plan you have chosen. The participating HMOs in your county can be viewed by selecting "HMOs and Rates by County."

Please mail your application to the HMO at the address listed.

Eligible applicants who submit a complete application to an HMO which is received by the HMO by the 20th of that month may be enrolled in the program by the 1st of the following month. If you have questions about the application process, you may contact the HMO to which you applied.

Individuals and sole proprietors will also be asked to provide documentation of their residence, household income, and employment status.

Keep in Mind…

Individuals, sole proprietors, and small employers in Cayuga, Cortland, Herkimer, Jefferson, Lewis, Madison, Oneida, Onondaga, and Oswego counties can take advantage of Healthcore, a Healthy NY pilot project that offers a 15% premium subsidy with an additional 5% subsidy for completing a confidential health risk assessment. Small businesses in Kings County may wish to apply to Brooklyn HealthWorks, a Healthy NY pilot project that offers a 19% premium subsidy. Coverage is provided by GHI. For more information on Brooklyn HealthWorks, visit www.brooklynhealthworks.com or call 1-718-596-4550.

Although Healthy NY benefits are the same at each HMO, the Healthy NY premiums do vary. So, check the price! Current premium rates can be seen on the HMO and Rates by County page or contact your local HMO.
